Permission

I, the parent of*

give the permission for my child to attend the Madrasah "Jungle Fun Trip" on Monday 20-Jul-2026 from 03:00 PM till 07:00 PM.

I give permission for my child to walk or ride in any vehicle designated by Madrasah Zeenatul Quran, its employees and volunteers, while participating in and travelling to and from this event.

Acknowledgement of Risk and Liability
I acknowledge that, while the Madrasah will take all reasonable steps to ensure the safety and wellbeing of my child during the trip, some activities may involve inherent risks.
I understand and accept these risks and give permission for my child to participate.
I agree that the Madrasah, its staff, volunteers, and organisers will not be held responsible for any injury, accident, loss, or damage arising from the normal risks associated with the trip.

In case of an emergency, I give permission for my child to receive medical treatment.

I give permission for photographs of my child to be taken and displayed on the Madrasah website, local newspaper or social media.

I agree and consent to all of the above stated.
